Training Record Card Objective

The OSH Center training record card provides quick access to a student’s official training transcripts. Simply scan the QR code on the front of the card to view the record. Alternatively, you can access transcripts through our View Student Training Record webpage.

What’s Displayed?

Each card lists selected purchased programs the student has successfully completed.

• If more programs are completed than can fit on the card, professional-level programs are shown instead of their related sub-programs.
• The selection process is automatic and cannot be customized.
• Note: Individual courses are not listed on the card.

How Many Cards Do You Receive?

• Each printed certificate program package includes one training record card.
• Example:
  • If you order two programs in one shipment, you will receive one card.
  • If you order two programs in separate shipments, you will receive two cards.
• Additional cards are available upon request by contacting our office.
